If you have dementia and your condition deteriorates significantly, you may not be able to wait for an appointment with your GP or community team.

Urgent Care Team Single Point of Access (SPA)

The Urgent 天美传媒 Response team provide urgent care to people in their homes, which helps avoid hospital admissions.

Admiral Nurses Dementia Helpline

The specialist dementia nurses will give free, expert advice and support, and are there for you when you need help.

Alzheimer’s Society Dementia Connect Support Line

If you are affected by dementia, worried about a diagnosis, or you are a carer, trained staff are ready to give you the support you need.

Continuing HealthCare (CHC) fast-track funding

If you have a rapidly deteriorating condition or are entering the final weeks of life, you may need ‘fast-tracking’ for immediate support of NHS continuing healthcare.

This is care funded and arranged by the NHS.
